Вплив дефіциту вітаміну D на стан репродуктивного здоров'я жінки

Анотація

В огляді наведено аналіз літературних даних про вплив дефіциту вітаміну D на репродуктивне здоров'я жінок фертильного віку. Розглянуто особливості перетворення неактивної форми, яка потрапляє в організм людини з продуктами харчування і під дією сонячного опромінення шкіри, на активну форму D гормону – кальцитріол (1,25 (OH) 2D). Показано, що провідною властивістю вітаміну D залишається активація процесів диференціації і проліферації хондроцитів і остеобластів, забезпечення фосфорно-кальцієвого гомеостазу, процесів мінералізації і росту кісток. Активні форми вітаміну D мають вирішальну роль і в інших біологічних процесах, включаючи регуляцію і диференціацію клітинного росту, активність метаболічних процесів, зокрема розвиток інсулінорезистентності, ожиріння, здатність змінювати нервово-м'язову провідність, впливати на імунітет, процеси запалення, гормональну активність репродуктивної системи жінки. Відзначено роль вітаміну D в процесах програмування розвитку плода та новонародженого, подальший ризик розвитку захворювань у дитинстві та дорослому житті завдяки знаходженню рецепторів до вітаміну D і 1a-гідроксилази в тканинах яєчників, матки, плаценти, а також в гіпофізі. Показано, що дефіцит вітаміну D обумовлює ряд несприятливих ускладнень вагітності: гіпертензію і особливо прееклампсію, збільшення частоти кесаревих розтинів і спонтанних передчасних пологів, бактеріального вагінозу на ранніх термінах вагітності, гестаційного цукрового діабету. Наведені дані про можливу роль дефіциту вітаміну D у формуванні синдрому полікістозних яєчників, його вплив на перебіг пубертатного періоду, статеве дозрівання дівчаток, порушення якого є предиктором формування полікістозного фенотипу в репродуктивному віці. Зроблено висновок про доцільність визначення рівня вітаміну D і його дотацію на прегравідарному етапі, що дозволить провести своєчасну профілактику прееклампсії, слабкості родової діяльності та оперативного розродження, а також буде сприяти народженню здорової дитини.
